Timely & Efficient Transitions
Reducing Avoidable Emergency Department (ED) Transfers
Current Performance: The Home's ED transfer rate remains below (better than) the provincial average.
Our Goal: Achieve a further 5% improvement this year.
What We Are Doing:
- This year, we will regularly review Emergency Department visits to understand the reasons for transfer and how we can better support care within the Home
- We will provide residents and caregivers with clear information about the care and services available in the Home, so they feel confident in the support options available
Equity, Diversity & Inclusion (EDI)
Expansion of Training Provided on EDI, Anti-Racism and Indigenous Cultural Safety
Our Equity, Diversity and Inclusion (EDI) Quality Improvement Plan focuses on creating a more welcoming, respectful, and culturally safe environment for everyone we serve and work with. This year, we will continue to build knowledge and awareness by supporting leaders to complete Indigenous Cultural Safety training and expanding EDI and anti-racism education to front-line staff through the first module of a new learning series. By making this education accessible and encouraging participation, we aim to strengthen understanding, reduce barriers, and support more inclusive, person-centered care experiences for clients, residents, families, and staff.
Safe & Effective Care
Reducing Falls
Current Performance: The percentage of residents who experienced a fall in the 30 days prior to assessment is 15.32%. This is above the target and provincial average.
Our Goal: Decrease the percentage of residents with falls from 15.3% to 14.5%.
What We Are Doing:
- This year, we will support staff with training and reminders to help ensure falls prevention strategies like bed height, call bells, and comfort needs are set up safely to reduce the risk of falls
- We will take time to better understand each fall so we can learn from it and put the right supports in place to help prevent it from happening again
- We will introduce additional support during the first 30 days after admission to closely monitor needs, identify risks early, and help new residents settle in safely
Prevention of Wounds
Current Performance: The rate of worsened stage 2 to 4 pressure ulcers remains above the provincial average.
Our Goal: To decrease the percentage from 6.41% to 4.90%, moving closer to the provincial average.
What We Are Doing:
- This year, we will ensure residents at higher risk for pressure injuries have personalized repositioning plans in place to support comfort, skin health, and prevention
- We will bring our care team together within 72 hours of any new or worsening pressure injury to understand what may have contributed and take timely action to support healing and prevent further issues
Reducing Use of Antipsychotics
Current Performance: We have seen consistent improvement over the past year. The percentage of residents taking a regular antipsychotic medication is 33%.
Our Goal: Reduce the percent of residents on these medications to 25%.
What We Are Doing:
- This year, we will continue building staff knowledge and skills through the Living and Working Well education series, helping teams use more non-medication approaches in everyday care
- We will strengthen regular medication reviews to make sure medications are still needed, used safely, and adjusted when appropriate to best support each resident's well-being
